Is it possible to add extension to the 2022 version of the program whose access to the Internet is cut off (isolated) by the our organization's firewall?

I don't think you can do that. Your firewall should allow access to PHPMaker's server in order to get update for template/extension files.
Can you tell the firewall port settings?
If you have proxy server and you use v2023, you may set your proxy server settings in advanced settings.

Then I doubt if you can do npm update, you may google "npm update firewall" for more information.
